REPORT A CONTRIBUTION FORM
This page will allow you to report a humanitarian contribution directly to FTS.
Please note that fields displayed with a "*" sign are mandatory.
European Union Member States:
Use the EDRIS reporting system instead. Reports entered in that system are automatically sent to FTS (for more information, see here).
CERF, CHF and ERF contributions:
This funding does not need to be reported to FTS through the form because FTS receives them directly through the CERF Secretariat, UNDP and OCHA.
Please fill in the project code if the contribution is applied to a Response Plan or Flash Appeal project.
